How do I create a custom firstpart to my country's name?

I have noticed a lot of countries that have been able to create custom names for the fist part of their country's name....for example, THE HOLY RUSSIAN ORTHODOX REPUBLIC of Imperial Russia...How is it possible to create my own custom name?

You need to have 500 million people in your nation before you get the custom title.
