Can you submit issues that get sent to every single player? If so, how?

Yes you can.
On the left hand side of the NationStates page, you know where the issues link is? Once you're in there, at the bottom of that page, there is another link. It gives you the option to submit a new issue.

But the odds aren't good that your issue would be submitted to every player. Players are eligible for issues based on the, for lack of a better phrase, condition of their nations.

Yes you can.
... once you reach 500 million in population. It takes about 3 months. Be patient.
Be aware that the vast majority of submitted issues are not used. There are typically about 1000 submitted for every one that Sirocco and the Issue Editors can use.

Also, user-submitted issues are only given to nations at least thirty days old (IIRC).
