Is the personal dentist in the fluoride problem a private dentist, or government-funded? And if you choose to fund the dentists, does that mean the government owns them/pays their wages or is just giving them money?

As I understand the issue (and its result), it means giving them higher wages straight from the treasury. So they'll still be expected to do dental work (otherwise they wouldn't be dentists ;) ).
The issue isn't clear on the specific status of the dentist you're talking to, but I don't see how that matters. The effect seems to be towards all dentists anyway.

I haven't looked (or received) that issue lately, but my guess is it's a private dentist, and the money from the government would be going to subsidize fluoridation, etc.
