I've noticed this on several occassions. An issue pops up (I'm guessing they are random) ignoreing the fact it isn't applicable to your nation.
By way of example, I got one today on youth crime. Since Day 1, I've put money into my police force, adopted mandatory military service, made skate parks, etc. And since Day 1, my summary has said "youth crime is unheard-of."
Now, though, I get an issue that says my youth crime statistics are terrible. I'm given a choice of giving more money to the police to enfore a curfew (doubtless raising my income tax) or doing nothing (doubtless raising my crime).
Just reflecting back, I'd say as many as 1/4 of the issues presented to me don't make sense when viewed against the daily history and summaries of my nation. Could there be exclusionary criteria so that issues which are contrary to a nation's well-established history are screened out?
