I have chosen the 'a crusade against barbaric religious practices has begun' option several times while my civil rights stayed unchanged at 'Outlawed'. However, this time, my civil rights rose from 'Outlawed' to 'Unheard of'. Is this a bug or a new sneaky change?
No. It fell from Outlawed to Unheard of. ;)

How about a cumulative effect? Like, in there being several numeric values to which a particular text label is applied, and you've finally left one zone and entered another?

No. It fell from Outlawed to Unheard of. ;)
Last time I saw the UN report for 'Most Liberal Nations', the nations at the bottom of the ranks had 'Outlawed' civil rights.
