Nation name: Chruatia
Was my puppet used in rp, I made only like 11-12 posts with it. Died some time ago, I revived it yesterday.
Now then I try to log in in forums I get message that its banned, and ban will be never lifted. Then I tried to verify e-mail again, it also gave message that its banned. So, I am strongly confused.

That's fairly common when you revive a nation and the forums are acting up. The game-to-forum link can't unban you if the forum registration database times out.

It's an easy fix via the Getting Help page, and you're now unbanned. If it happens again or to anyone else, make your request that way.
