However the titles are as follows:

The Holy Father
This is the supreme ruler of Isam. He holds office for life and is elected by the Church Council upon the death of the previous Holy Father.

The Seceretary to the Holy Father
This person normally takes care of the day-to-day administration of the Holy Empire of Isam and its Protectorates. At present the current Seceretary is the real ruler of Isam as the Holy Father is elderly and frail - however he will always get permission from His Holiness before making any major decisions.
The Seceretary is appointed by the Holy Father with the approval of the Church Council and holds office for a decade.
