I have yet to work out how these sort of designers ever make any money, because who in their right mind, would pay good money for that? It's utterly pointless.
Simplification: Money they make from clothing is made mostly from their prêt-à-porter collections and haute couture nowadays is more of a PR thing to raise/maintain the image of the house making it - to display how innovative and inspired and inspiring they can be. That is not to say that haute couture is not sold - it is, but it tends to be modified to customer desires and tones down the exaggeration of the haute couture seen on the runway (in fact, according to the French legal definition of it, the design must be made-to-order for private clients that require one or more fittings). Also, a lot of haute couture is not as extravagant as the sample shown in the OP and can be marketed to an exclusive segment without significant adaptation.

It's pretty hillarious.
But I'm pretty sure most fashion shows feature a few outfits that no one with an ounce of taste, self-respect, or body fat would wear. Their purpose is not to make it onto the fashion marketplace, but to get attention. A few bizarre outfits make the viewers think "this person is daring and sooo avante garde." It makes them feel less banal for then cheering the next line of mediocre crap that gets put into circulation.
Edit: As much as it looks like it, this post was not intended to be a response to the one immediatly above it. But I do wholeheartedly agree.

when it's at home? is a (chiefly) antipodean colloquialism. It's just a way of speaking when you want to know what something is or who someone is.
In this case he was asking what the two terms meant, and Kat answered him.
I think the outfit might have looked better on a woman who weighed enough to have tits to hold the buildings up. But that's just me - I have a thing for women who aren't so skinny their ribcages can be used for xylophone practise.
Both the dress and the model wearing it are teh ugleh.
