I've been going to ytmnd.com for awhile now, but I only just recently tried my hand at making them. Some of you may recall the inspiration for my first one... :D

There's a link (http://ytmnd.com/sites/new) to the YTMND-o-matic on the main page, though I'm pretty sure you have to register before you can create a site.
As for actually creating a page, you just upload a still image or gif animation, a wav or mp3 sound file, and some text. I'm still not quite sure how to make a gif animation, though (yes, I did pilfer that Colbert animation, but it was from a friend whose website I credited).

Funny story. Back in the early days of Gabbly, when people were really excited and lots of people were on it at all hours, we noticed a trend on the forum. Three, four, five topics, each with one of the deadly sins in their title. We decided to "help" the trend along, so Dalek posted two or three threads containing the remaining sins, and then I got a screenshot of it.
The original thread is here (http://forums.jolt.co.uk/showthread.php?t=499010), with a link to the original shot (I trimmed it down some to make the YTMND).
