If we're talking about materials that are stable at room temperature and pressure, then it's the heavy metal osmium. It's "man-made" in the sense it has to be purified. You can't go much denser from this, because density is determined by the electronic structure of the atoms of the metal. You can't customize the electronic orbitals - they're naturally what they are, and can't be changed.

But if we're talking about unstable materials, then there are at least these:

the "simulated sun" experiment where bullets are shot at each other to recreate the conditions in the sun
fusion reactor fuel compressed with laser beams from 91 meter-long lasers
heavier transuranium elements.

None of these persist for more than a split second.

In particle accelerators, even more dense states of matter can be produced. These involve only a few atoms, so it's questionable if it's "material" at all.

If you made a bullet out of Chuck Norris and Mr. T's gold chain it would chew through Superman rather easy. The problem is finding a gun that could actually fire it.

hmmm, I think you will be hard pressed to find the hardest man made substance as the stuff that we make that is dense (diamonds) has a natural variant, and the stuff we make that doesn't have a natural variant is not particularly dense (plastics, ceramics, textiles).

As far as the densest natural substance, you would have to go with the material a black hole is made of.

Actually its not 0 volume as the event horizon has a definite diameter, and there is a minimum amount of mass needed for gravity to overcome the repulsion force of the atoms. Thus you have total area and total mass, with a defined density, although density will vary (am I wrong?). Anyhow, second place would be the material of a neutron star, a gram of neutron star would crash through the floor and head straight for earth's core.

Depleted uranium is super dense. Though anything higher is harder to produce on a large scale.
