Latin grammar help

Can you take the nominative noun phrase "Apocalypsis mittens" ("revelation sent") and put it into the correct grammatical noun declension so it becomes a dedication, like "to the revelation sent". Thanks!!!

PS By dedication I mean if one dedicates i.e. a book to something. So "Dedicated to Mary" is a different sense from "going to Europe."

or is it accusative?

ad apocalypsem mittentem

I think it's dative though
