That depends on what your definition of religious leader is. Since I see that section 2 assumes "religious leader" is the person in charge of deciding service times, Ill go with No for now.

For now I attend the Saturdays at 6:00pm service though sometimes I attend the morning services, depends on my current schedule.

7. Any further comments? There are several Catholic mass times and churches in my area I could attend. The services range from Saturday evening to Saturday night, Sunday morning, Sunday afternoon, Sunday evening, and even Sunday night.

Are you a religious leader? Yes and No I am not an Imam, but have every right to study and teach Qur'an. However, for the purposes of this survey, I shall skip section 1.
Section 2

There are 5 daily prayer times: Fajr (pre-dawn), Dhuhr (afternoon), Asr (pre-evening), Maghrib (pre-sunset), and Isha (night). Isha is generally the most widely attended since many people around here have day jobs.

Because they are the times given by Allah in Qur'an. It is not my place to change them.

Your questions seem very pointed to Christianity and its religious practices. I would think the world of psychology would be better prepared for other ways.

6. Why? I'm freakin' lazy

13. Why or why not? My duty in this situation is to the kids before myself, so my social circle has to wait from time to time :D

Yes, I think 11:00 is a good time to hold a religious service. Late morning is always a good idea. It also leaves the rest of your day free.

My church is starting a 7:00 PM service that I might try attending for a while. See how it goes.

We aready have, the shift to two services was recent...

The primary reason was growth of membership and attendance..

Yes, I'm presbyterian.... In the end, it is the congregation as a whole, by vote, who approve changes such as that...

It's a point to representative church government as found in Presbyterian and Reformed churches... Leadership is elected, and most major decisions are handed by the body itself, with recommendation from the Session (body of Teaching and Rulling Elders, elected by the Congregation; whose purpose is to handle the spiritual wellbeing of the congregation...) and Diaconate (body of Deacons, whose purpose is the physical wellbeing of the congregation, and decisions regarding grounds upkeep...)
