Does Joey Ramone have a lisp?
I'm positive he does.
While I admit it's intermittent and unreliable at best, (he can pronounce the "tz" in "Blitzkrieg" but not the "t" in "back beat" or the "st" in straight line", etc. in Blitzkrieg Pop. I'll try and find more examples, if anyone's interested...

Two reasons why not: One, because it was just a strong street accent, and two, he's dead and we'll never know. :)

At times it does sound like he has it and other times it doesn't. I think it's just his accent.
Only when he's singing, and then, only sometimes. Watch interviews and footage --- When he talks, he sounds fine. Unless he's drunk.

While alive though, NO, Joey Ramone didn't have a lisp. Just a horrible case of marble mouth, like anyone from Queens.
