Give this a read. For pity sake, some people just can't be trusted with children. What's so hard about "Don't leave a baby in the car while you pop to the butchers"?

People make mistakes. Frequently even mistake prone people decide to have have children. Indeed, mistake prone people are prone to have children without even deciding too. That's what happens. There is no should about it, as no one has control over other people's reproduction. If you're asking if the government or some other group should have control over people's reproduction, perhaps you should start another thread asking that question. Otherwise we should just accept that life entails some degree of risk and the children of the mistake prone will be at more risk than others. If it bothers you, we can try to reduce the risks to these children via education, developing robot nannies, etc.
