I would assume being burned alive (such as in a house fire) would be up there somewhere. Having the male reproductive organs crushed would seem a likely suspect as well. I could probably come up with more, but I don't really see much reason to contemplate incredibly painful things. I have enough pain without imagining more, no disrespect intended of course. o.O;
