I've had issues pending for about a year now, doesn't matter if I dismiss them or try to select an option, they never seem to go through. I've tried the trick of moving regions to no avail. I've decided I'd quite like to answer some of the newer ones, so, any idea how to fix it? Cheers.

Change your issue frequency from 'none' to some number. Issues don't process unless you're getting new ones.
I've cleared your pending issues.

Did that fix include processing issues for nations in vacation mode? I had two puppets with 6 issues pending in vacation mode but they haven't got any left.

It seems to have done so: One of my own puppets that I checked yesterday had an issue listed as answered three days ago, in its recent history, although it's been in vacation mode for months...
