I'm at 410 million people after about 2.5 months and all of the issues I have seen have both extremes and no middle ground. Some days I don't mind being extreme but there are several issues where I had better solutions but not the choice that could match it. is this by design?

Yeah, pretty much. This game was largely designed as political satire, and there's a definite shortage of middle of the road options.
