Okay, so a long time ago, I created a custom flag for my nation, and I used it. I have created another custom flag, but this time, whenever I upload it, the old one still shows. Ok, so I refreshed and cleared the history, cache, cookies, etc.. Tried again, the same happens.

So I tried uploading a real country flag and THEN changing it to the newly custom made flag. When I come to upload the new custom flag, the old one shows up again!

Whats going on? any help appreciated, thanks :)

Either you're re-uploading the old flag or your cache is not clearing properly. If you're using Firefox, right click on "Leque" on the left hand side of Nationstates.net and choose "Open in New Tab." In the new tab, press Ctrl+Shift+R. This forces Firefox to reload that individual page and its content, and should refresh your flag if it is uploaded properly.

Don't suppose you're using a web accelerator...
