So, all we do is decide on the issues?
The core of the original game is, indeed, to decide on issues and see how they affect your nation. However, there a many other things you can do (these aren't listed in any specific order, just the order in which they came to mind).
1. You can join the UN, and participate in proposal drafting and debate.
2. You can join the UN, and take part in the invasion game.
3. You can take part in Roleplay, on the NationStates and International Incidents forums.
4. You can take part in discussion and debate on a huge variety of topics in the General forum.
5. Once you've reached 500 million, you can attempt to submit an Issue yourself.
Issues and Resolutions are among the few ways of getting your nation's name coded into the game for posterity.
Most people who continue playing the game take part in at least one of the above. Some try to do all of them. Each has interesting things about it which aren't discovered until you have a good go.

You could also join one of many regions and they might have their own events that you might find interesting.
