Voting is compulsory, the mining industry is making inroads into environmentally sensitive areas, citizens can be frequently spotted going about their business stark naked, and voting is voluntary.

I've noticed the size of the government can be inconsistent, too.

Nope, no bug, you simply got the "Should Democracy Be Compulsory?" issue twice, and chose different options. If you dislike it, just remember to be consistent on how you answer issues.

Hmm. I'd have expected a flag that was set depending on how you answer, and that to cause the appropriate text to be inserted. Obviously my expection was too elegant ;)

More fun: it seems to be implemented as text values, not as an index to the textual values in the issues. In some cases the effect of this can be observed, like when a currency value is part of the text event and the currency is changed after the issue is passed, or when [violet] fixes a typo in the text of the effect. It will then state the incorrect (old) currency or maintain the typo.

Heh... I remember, back last summer, when someone managed to answer the Spiritual Advisor issue 3 times in a row, and picked a different option each time, just to have the government being staunchly Catholic, advised by a new-age guru, while also being avowedly atheist.
