Hi, yesturday I got 2 issues and replied to both of them, I though the option was quite fair but now, when before i was 12th in my region rank now i'm 53rd, just wondering, if when you dismiss an issue will anything decrease? And also is there a time when i can change my previous option?

Dismissing an issue does absolutely nothing to your country and just wait for it to come again, :)

Since the category of the UN report changes daily, it is normal for your nation to rank differently each day.
You can change your decision on an issue until it is implemented, which happens during a system update. At that moment you'll also get a new issue.
